Question to the Secretary of State for Health on the 'blame culture' in the Messenger Review

I am very concerned that the Messenger review found evidence of a 'blame culture and responsibility avoidance in the health service. We have to be clear that this culture is not just damaging but actually kills patients because lessons are not learned from mistakes that are actively hidden. I asked the Health Secretary what we can learn from the airline industry, which adopted a no-blame culture and, through that, dramatically reduced airline-related deaths.
